YDC FORMS AND REGISTRATION
Please complete the following steps so that we can confirm your registration in the YDC Program.
1. Client and at least one parent (if client is a minor) must schedule a free, 15-minute initial consultation by clicking here.
2. After the initial consultation, click on the "Program Registration" button to provide us with your contact information.
3. Download and sign the Coaching Agreement, Cancellation Policy, and Informed Consent documents. They should be signed by the client and responsible parties (if client is a minor), and emailed to vanessa@youthdiscoverycoaching.com.
4. Remit payment by clicking on the corresponding button.
Once these steps are completed you will receive an email confirming your registration.
